This checklist is completed by staff in several areas, and includes the following sections which align with the phases of establishing and assessing a potential/current partnership:
- Initial conversation/prospecting
- Agreement/partnership formation
- Partnership evaluation
In this document, the applicant addresses/reviews the partner's history of working with the organization, commitment to racial equity, program alignment, contact information, cost, MOU/contract elements, and services (including communication, timeliness, resident satisfaction, and diversity & inclusion).
