Provided by Operation Pathways, an affiliate of The NHP Foundation, this Community Scan, one of the threshold requirements for the CORES Certification, is a review or identification of the existing local amenities, services, community assets, potential partners, and other resources in the larger community. The Community Scan must also include an assessment/analysis of the quality of these resources and services and should be conducted prior to the delivery of services and programs. It is expected that the Community Scan is updated at least every 6 years.
