Provided by Operation Pathways, an affiliate of The NHP Foundation, this assessment is a great example of one of the threshold requirements for the CORES Application. The Resident Opportunities & Priorities Assessment should be updated at least every 3 years and include the following:

• Analysis of the aggregated demographics/data available for the residents of the property
• Direct feedback (e.g. interviews, focus groups, etc.) from the residents
• Explanation of how the information in the assessment was obtained
• Analysis showing what was learned
